Report a child absent

All absences are considered unexcused until we receive an excuse note with the completion of this online form. Parents must bring a note or use this system within three days of the last absence. All information must be filled out on the email system to make the excuse valid.

State of Florida approved absence reasons include:

a. Illness of student. Illness that lasts more than 3 days must have a doctors note to be excused.

b. Major illness in the immediate family of the student (immediate family is determined as parents, brothers, sisters, grandparents, aunts, uncles, legal guardians or persons in loco parentis, or a member of one’s own household).

c. Death in the immediate family of the student.

d. Religious holiday of the student’s faith (including Yom Kippur, Rosh Hashanah, Epiphany, etc.)

e. Religious instruction (including Bar/Bat Mitzvah, Confirmation, etc.)

f. Religious institutes, conferences, or workshops, provided that the principal or designee approves the absence in advance.

g. Absences for trips or other parental requests as judged appropriate for the student by the principal. These must be approved at least one (1) week in advance by the principal or designee. Please email Mrs. Steinacker for excused absence approval of trips.

Families whose students have more than 6 unexcused absences in a quarter may be referred to our school Social Intervention Team. Use of this Student Absence Reporting Form below indicates that you understand the description above.

Does the student have new or worsening onset of any of the following symptoms? · Fever (greater than 100.4) or chills, Muscle or body aches, Fatigue, Shortness of breath or difficulty breathing, Headache, Congestion or runny nose, Sore throat, Nausea or vomiting, New loss of taste or smell, Diarrhea, or a New uncontrolled cough (a change from your regular cough)·

If YES, the student cannot come on campus until student is 24 HOURS SYMPTOM AND FEVER FREE without the use of OTC medicine. Please refer to the student screener at: https://www.pasco.k12.fl.us/coronavirus/screeners
