Parents, When you complete this form, you are changing the way your child is home for the selected date. One form needs to be completed for EACH child that is going home a different way. For example: If you have a Kindergarten student and a 2nd grader, two forms need to be completed; one for the information for your Kindergartener, and one for the information for your 2nd grader. If you make a change "From Now On" this information will be entered in our system. Changing from Bus to any other method of transportation "From Now On", will result in loss of bus. EACH child that is going home differently needs their own form filled out. This is for the safety and security of all children. You will receive an email reply confirming that we have received the transportation change. If you do not receive an email, OR you are completing this form AFTER 3:00 PM for a same-day transportation change- please call Oakstead at 813-346-1500 to ensure that your child goes home the correct way. In order to make sure all children are safely dismissed, we cannot honor changes made after 3 pm.
If you make a change "From Now On" this information will be entered in our system. Changing FROM Bus TO any other method of transportation "From Now On", will result in loss of bus for the rest of the year.
